Your personal information disclosed is required exclusively for the treatment services provided. Please address and concerns related your privacy rights and confidentially.

What is Personal Information?

  • This includes information that relates to your personal characteristics (e.g. gender, age, address, contact information, education, family circumstances)

  • Medical Information (eg. diagnosis)

How is personal information Secured?

  • Paper information is either under supervision or secured in a double locked area.

  • Electronic hardware is either under supervision or secured in a double locked area. Encryption and passwords are used on all electronic devices.

Circumstances where Privacy Information is Released

There are certain circumstances where there is a legal obligation to disclose information to the appropriate authorities. These include:

  • When an individual is at risk of serious harm to themselves or others

  • If subpoenaed by the legal system

  • Is a member of a vulnerable population is suspected of being abused

  • Quality assurances measures by regulatory body
